The claimant should give "Pacific's"
third party administrator immediate written notice as soon as reasonably
practicable and in any event within 30 days of the claim. A
completed Notification of Claim Form and respective claim
documents must be submitted to the third party administrator no more than
90 days after the claim incurs.
Failure to meet with the time limit may lead to denial of a
claim.
